Procedure causes faulted state and shred service to stop

Hi ,

I've just noticed an unusual fault on the Ris ....I currently have RIS and imageserver on one server2008 r2 box running sql2008

 

When I place a new order and use the IHE  procedure "Chest x-Ray " (this is needed for protocolling ?),then click on save at the end of the order I get the dreaded service is in a faulted state ..when I check on the RIS server I notice that this has actually caused the Ris shred service to stop .

If I restart the service and then wait a few minutes and cancel and replace the order

with a "CTabdomen Pelvis" then it's fine in fact it's always fine when placing the CT orders but Chest x-ray crashes the RIS every time .

The system is sitting with only default sample data at the mo as I anted to thoroughly test it first ...any  thoughts on why the order would cause the Ris shred host service to crash with faulted state communication errors ?

thought I'd better add a bit more info to this so I've attached the client & server logs

and also screenshot of error .I did try delete the procedure and try another one I made up for Xray but still same error .

The eror doesnt seem to appear on NM pertusion and CT procedures .

The db is sitting with only sample data at the mo .

Hi FT,
The error in the client log file is occurring at 2010-05-28 20:35:40, but your server log file does not cover this time. The server log file will contain the important information, so I would suggest finding the server log that covers the same moment in time.

Hi Jonathan ,

 

I think because the shredservice stopped the logging also stopped which is prob why it's missing ,just as a matter of interest has anyone loaded Ris2.0 inc sample data and tried booking in an new order using the Chest X-ray procedure ?

I think the same errror may occur if you try using that procedure on a fresh install ..I also tried booking random orders for a patient ..I booked 20 in all ...but a random generated Chest X-ray procedure wasnt generated at any point ..I think it may be down to sample data included with package ..maybe someone else can also confirm ?
